How does NHSTC, Inc. home healh care work?

First, a NHSTC, Inc. registered nurse (RN) Supervisor visits with you and assesses your child’s unique needs. Next, we work closely with you and your child’s physician to create a written plan of care. We carefully match your child’s specific needs with a NHSTC, Inc. pediatric caregiver whose skills and experience can best support your child. Your RN Supervisor orients the care team to your child’s needs and makes regular in-home supervisory visits. Over time, we continually reassess your child’s needs and update your caregivers training accordingly so they can continue to provide exceptional care for your child.

What is a Plan of Care?

A nursing Plan of Care outlines the nursing care to be provided to you. It contains a set of actions that our nurses will implement to resolve nursing problems identified by your doctor’s assessment. The Plan of Care guides in the ongoing provision of nursing care and assists in determining if our care meets the prescribed goals.
